Ekspresi E-cadherin dan Matrix Metalloproteinase-9 pada Invasi Miometrium Endometrioid Adenokarsinoma Endometrium

Abstract

ABSTRAK Latar belakang Invasi adalah petanda biologi tumor ganas yang berperan dalam proses metastasis. E-cadherin merupakan mediator hubungan antar sel. Matrix metalloproteinase (MMP)-9 merupakan salah satu enzim protease yang berperan dalam degradasi lokal membran basalis serta jaringan ikat. E-cadherin berperan penting dalam adhesi antar sel epitel dan mekanisme arsitekstur jaringan, sedangkan MMP-9 berperan dalam menciptakan jalur untuk migrasi sel tumor. Ekspresi E-cadherin dan MMP-9 berhubungan dengan potensi terjadinya invasi ke dalam miometrium pada kasus endometrioid adenokarsinoma endometrium. Tujuan penelitian ini untuk menganalisis ekspresi E-cadherin dan MMP-9 terhadap invasi miometrium pada endometrioid adenokarsinoma endometrium. Metode Rancangan penelitian yang digunakan adalah cross sectional. Sampel dibagi menjadi 2 yaitu invasi kurang dari setengah ketebalan miometrium dan invasi lebih dari setengah ketebalan miometrium pada penderita endometrioid adenokarsinoma endometrium yang didiagnosis di RSUD Dr. Soetomo mulai Januari 2010-Desember 2011 yang telah memenuhi kriteria inklusi. Sampel dipulas imunohistokimia dengan antibodi E-cadherin dan MMP-9 lalu dinilai secara semikuantitatif. Perbedaan ekspresi E-cadherin dan MMP-9 terhadap invasi miometrium pada endometrioid adenokarsinoma endometrium dianalisis menggunakan uji Mann-Whitney. Hubungan antara ekspresi E-cadherin dan MMP-9 pada endometrioid adenokarsinoma endometrium dianalisis menggunakan uji Spearman. Hasil Analisa statistik ekspresi E-cadherin dan MMP-9 terhadap invasi miometrium pada endometrioid adeno-karsinoma menunjukkan hubungan bermakna (p<0,05). Ekspresi E-cadherin dan MMP-9 pada endometrioid adenokarsinoma endometrium menunjukkan perbedaan tidak bermakna (p=0,125). Kesimpulan Ekspresi E-cadherin menurun, sedangkan ekspresi MMP-9 meningkat pada endometrioid adenokarsinoma endometrium. Kata kunci: E-cadherin, endometrioid adenokarsinoma endometrium, invasi miometrium, MMP-9. ABSTRACT Backgroud Invasion is biologic hallmarks of malignant tumor and initiates the metastatic cascade. Dissociation between cells are mediated by the E-cadherin, family of glycoprotein. Matrix metalloproteinase (MMP)-9 is enzyme protease that mediate the local degradation of the basement membrane. E-cadherin has a important role in the epithelial cells adhesion and mechanism of tissue architecture, while MMP-9 has a play role in migration cells. The expression of E-cadherin and MMP-9 correlates with the potential for myometrial invasion in endometrioid adenocarcinoma endometrium. To explain E-cadherin and MMP-9 expression in myometrial invasion of endometrioid adenocarcinoma endometrium. Methods Study design was cross sectional. Sample were divided into 2: invasion less than half and more than half of myometrium thickness in endometrioid adenocarcinoma endometrium which had been diagnosed on Dr Soetomo hospital Surabaya at range between 2010-2011. Sample has been staining with immunohisto-chemistry with E-cadherin and MMP-9 antibody and has been evaluated semiquantitatively. The differences of E-cadherin and MMP-9 expression on myometrial invasion endometrioid adenocarcinoma endometrium were analyzed by Mann-Whitney. The correlation between E-cadherin and MMP-9 expresssion in endometrioid adenocarcinoma endometrium were analyzed by Spearman. Results Statistic analyses of E-cadherin and MMP-9 expression on myometrial invasion endometrioid adenocarcinoma endometrium showed significant differences (p<0.05). There were no correlation between the expression of E-cadherin and MMP-9 in endometrioid adenocarcinoma endometrium (p=0.125). Conclusion E-cadherin expression was decreased, and MMP-9 expression was increased on endometrioid adeno-carcinoma endometrium.
